Please print and fill out the applicable form (PDF) and send to CFI as indicated.

Form A is for employees represented by CFI in one of the four regional bargaining units. No payment is necessary as dues are paid through payroll deductions. Membership in this category includes professional division and union membership.

Form B is for independent contractors, students or others interested in the profession who wish to become members of the professional division of CFI. There is an annual fee as described on the form. Professional division membership includes discounted rates for our continuing education events and the option to be included on our referral list.
